Kozak nee Bananch, dear father of Audrey (Allen) Brown, Ricard Kozak, Jr., Lloyd (Diane) Kozak and Elizabeth (Michael, Jr.) Dobler; cherished grandfather of Stephen, Travis, Kristyne, Jessica, Jeremy, Justin, Khileigh, Brianna, Megan, Michael Jr., and Samantha; great grandfather of Shawn, Nicholas, Bentley, Levi, Wyatt, Cody, Tegon, Colton and Brantley; fond brother of the late Robert Kozak; brother on law of Adeline (Joe) Truty, Edward (Vicky) Banach and Herbert (Deb) Banach; uncle of many nieces, nephews, great nieces and great nephews.
